Patient Consent
Effective July 13, 2026
By confirming a diagnostic booking, the patient or authorised representative consents to collection and processing of the sample for the selected tests.
Collection and testing
I consent to home sample collection by assigned personnel and laboratory processing by the assigned partner lab.
Information sharing
I authorise ShriCare to share the minimum necessary identity, contact, address, clinical booking and payment information with fulfilment partners.
Preparation and accuracy
I confirm that patient details are accurate and that preparation instructions, including fasting when applicable, will be followed.
Reports and follow-up
I understand that results may require clinical interpretation and that ShriCare does not replace a treating doctor or emergency service.
Withdrawal
Consent may be withdrawn before collection by contacting support. Withdrawal after collection may not stop processing already underway and may affect refund eligibility.
